Social Worker Responsibilities & Duties

Conduct psychosocial assessments for new residents

Develop individualized care plans in collaboration with healthcare teams

Provide individual and group counseling to residents

Assist residents in adjusting to nursing home life

Advocate for residents' rights and needs

Facilitate communication between residents, families, and staff

Coordinate discharge planning and follow-up services

Assist families with understanding and navigating the healthcare system

Organize and implement recreational and therapeutic activities

Maintain accurate documentation and case records

Participate in interdisciplinary team meetings
-
Offer crisis intervention services as needed
-
Provide support and resources for end-of-life care

Ensure compliance with federal and state regulations

Conduct family meetings and provide mediation when necessary

Coordinate with community resources for additional resident support

Educate staff on psychosocial issues affecting residents

Monitor and evaluate the effectiveness of care plans

Develop and implement policies to improve resident care
Social Worker Qualifications & Skills
-
Master’s degree in social work (MSW)
-
Licensed Clinical Social Worker (LCSW) credentials
-
Experience in geriatric or long-term care settings
-
Knowledge of Medicare and Medicaid regulations
-
Strong communication and interpersonal skills
-
Experience with case management and care coordination
