HOUSE SUPERVISOR- DEVELOPMENTAL DISABILITIES
Job Description:
A House Supervisor shall provide Person-Centered Services through implementation of individual program plans, program policies, and procedures. Direct Care Staff are responsible for assisting persons served in maintaining and\or increasing independence in their home. The House Supervisor is responsible for assuring that all rules relevant to the delivery of 245D are being implemented. This position is supervised by the QCS Owners.
Qualifications: 1-2 year related experience working with persons with developmental disabilities, valid MN drivers license is required and employees must meet professional requirements of freedom of any criminal and chemical abuse activity. Prior supervision experience is preferred.
Major Responsibilities:
- Provide direct care and independent living skills training to service recipients.
- Assess, develop and monitor service recipient services.
- Effectively communicate verbally and in writing to appropriate persons/agency as required.
- Public relations/Community Visibility.
- Provide effective supervision to direct care staff.
- Assuring that all shifts are filled.
Hours:
- Monday- Thursday 6 am- 4 pm or Monday-Friday 6am-3 pm
- On-call every other weekend

Job Duties:
- Provide 100% personal cares to four ladies with cerebral palsy, including toileting/bathing and transferring with hoyer lifts
- Provide recommendations or modification to service recipient plans as needed to interdisciplinary team members.
- Completes service recipient’s daily documentation thoroughly, accurately, and in a timely manner.
- Document service recipient progress as required.
- Provide training, counseling, and assistance to service recipients in daily living, including but not limited to: Personal Hygiene/Grooming, community Integration, care of clothing/Belongings, service recipient Rights, shopping, socialization, meal planning/preparation personal care, money management, housekeeping, health care
- Maintain healthy meals for service recipients- grocery shopping.
- Assure that service recipients’ rights are observed.
- Participate in all 245D required training
- Report and document all accidents, incidents and vulnerable adult concerns.
- Communicate responsibly with co-workers, case managers, guardians, and others involved in the lives of the service recipients in order to provide the highest quality of care.
- Computing payroll/time and attendance of staff
- Manage direct care staff: Including: scheduling, training, coaching, writing and conducting performance reviews, interviewing, staff disciplinary actions.
- Conduct monthly staff meetings
- If a House Supervisors cannot find someone to fill the shift, they are responsible for working the shift.
- Maintain and enforce the service recipient activity calendar.
- Schedule and transport all service recipients to ALL doctor appointments.
- Other duties assigned by management.

Job Type: Full-time
Pay: $25.00 - $30.00 per hour
Expected hours: No more than 40 per week
