Position Summary:
ForWARD Health is seeking an experienced Registered Nurse (PRN) with a minimum of two years of clinical experience and experience in OASIS documentation to join our compassionate home healthcare team. This role is designed for a highly skilled and flexible professional ready to deliver exceptional care to patients in their homes.
Key Responsibilities:
- Conduct comprehensive patient assessments, ensuring accurate and detailed OASIS documentation to meet regulatory requirements.
- Develop, implement, and regularly update individualized care plans in collaboration with patients, families, and interdisciplinary team members.
- Deliver skilled nursing care, including but not limited to medication administration, wound care, IV therapy, and monitoring of chronic conditions.
- Educate patients and families on health management, disease prevention, and ongoing care needs.
- Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal health outcomes.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated, high-quality patient care.
- Maintain compliance with Medicare, Medicaid, and all state and federal regulatory guidelines.
- Provide on-call support as needed, addressing urgent patient care needs promptly.
Qualifications:
- Current and unrestricted RN license in Colorado.
- At least two years of nursing experience, preferably in home health or a related field.
- Proven proficiency with OASIS documentation and understanding of its role in home healthcare.
- Strong knowledge of Medicare and Medicaid guidelines.
- Proficiency in EMR systems for documentation and record-keeping.
- Valid driver’s license and reliable transportation for home visits.
- Excellent communication, critical thinking, and organizational skills.
Preferred Qualifications:
- Certification in Wound Care, IV Therapy, or a related specialty.
- Familiarity with AXXESS .
- Bilingual skills are an asset.
Physical Requirements:
- Ability to travel within the service area to conduct patient visits.
- Capability to lift and transfer patients safely when needed.
Schedule:
This is a PRN (as-needed) position with flexible hours, based on patient needs and availability.
