Primary Goal of the Position
Develop new referral sources, strengthen existing relationships, and increase qualified patient referrals for Sunflower Home Care's Home Health and Personal Assistance Services while maintaining ethical and compliant marketing practices.
Position Summary
Sunflower Home Care LLC is seeking a motivated, outgoing, and professional Community Liaison / Marketing Representative to help grow our Home Health and Personal Assistance Services programs.
The primary responsibility of this position is to develop and maintain relationships with physicians' offices and other healthcare and community referral sources. The Community Liaison will regularly visit referral partners, educate them about our services, follow up regarding referred patients, and help identify opportunities to connect individuals in the community with appropriate home-based services.
This position is ideal for someone who enjoys building relationships, being out in the community, and helping grow a healthcare organization.
Key Responsibilities
- Visit physicians' offices and clinics on a consistent weekly basis to develop and maintain professional relationships.
- Educate physicians, medical assistants, office managers, referral coordinators, and other healthcare professionals about Sunflower Home Care's services.
- Develop new referral relationships and maintain communication with existing referral sources.
- Follow up with referral sources regarding referrals and the patient's care, while following HIPAA and privacy requirements.
- Communicate with the agency's clinical and administrative team regarding new referrals and referral-source needs.
- Follow up on potential referrals and help ensure referral sources receive timely communication.
- Identify opportunities to increase referrals for:
- Skilled Nursing
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Home Health Aide services
- Personal Assistance Services / attendant care
- Build relationships with additional community referral sources, including:
- Hospitals and discharge planners
- Rehabilitation facilities
- Skilled nursing facilities
- Assisted living and senior living communities
- Case managers and social workers
- Senior centers
- Community organizations
- Medicaid and healthcare organizations
- Attend and participate in health fairs, senior events, community events, networking events, and educational activities.
- Assist in organizing agency-sponsored community outreach events.
- Distribute approved marketing materials to referral sources and community partners.
- Maintain an organized schedule of marketing visits and referral follow-ups.
- Document marketing activities, contacts, visits, referrals, and follow-up efforts.
- Identify new opportunities and organizations where potential patients may learn about our services.
- Represent Sunflower Home Care in a professional, ethical, and positive manner.
- Follow all applicable HIPAA, Medicare, Medicaid, Texas HHSC, and healthcare marketing requirements.
- Never offer or provide improper incentives, payments, gifts, or anything of value in exchange for patient referrals.
Qualifications
- Previous experience in home health, home care, healthcare marketing, community outreach, sales, or provider relations preferred.
- Strong communication and interpersonal skills.
- Comfortable visiting physician offices and speaking with healthcare professionals.
- Ability to build and maintain long-term professional relationships.
- Self-motivated and comfortable working independently in the community.
- Organized and able to consistently follow up with referral sources.
- Professional appearance and communication.
- Basic computer, email, and documentation skills.
- Knowledge of the El Paso healthcare community is a plus.
- Bilingual English/Spanish preferred.
- Valid driver's license and reliable transportation required.

Pay: From $20.00 per hour
