Duties
- Review and verify client documentation to establish eligibility for Medicaid and other health insurance programs
- Conduct thorough assessments of applicants’ medical and social histories, including hospital experience and behavioral health status
- Collaborate with case management teams, social workers, and healthcare providers to facilitate discharge planning and coordinate services
- Utilize electronic health records (EHR) systems for accurate data entry, documentation, and tracking of client information
- Maintain strict adherence to HIPAA regulations by safeguarding patient confidentiality during all interactions
- Communicate effectively with clients from diverse backgrounds, including bilingual populations, ensuring clear understanding of eligibility criteria and next steps
Qualifications
- Proven experience working with individuals in healthcare settings, preferably with hospital or acute care experience
- Familiarity with electronic health records (EHR) or electronic medical records (EMR) management systems
- Excellent customer service skills combined with compassionate client communication abilities
- Ability to perform detailed data entry accurately while managing multiple priorities efficiently
- Knowledge of HIPAA compliance standards and maintaining patient confidentiality at all times

Pay: $18.00 - $21.00 per hour
