Position Summary
The Home Health Caregiver provides non-medical, in-home support services to individuals receiving Colorado Medicaid. Services are delivered in accordance with the client’s authorized Service Plan and may include homemaking, personal care, and health maintenance tasks. Caregivers support client independence, dignity, and safety while complying with all Colorado Department of Health Care Policy & Financing (HCPF) requirements and agency policies.

Essential Duties and Responsibilities
Provide in-home support services in accordance with the authorized Service Plan, including:
- Homemaking Services: Perform light housekeeping, laundry, meal preparation, grocery support, and basic home organization; maintain a clean and safe environment and report safety concerns.
- Personal Care Services: Assist with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, transfers, ambulation, and continence care while promoting client independence, dignity, and privacy; follow infection control standards.
- Health Maintenance Tasks (as authorized): Assist with permitted health maintenance activities, including medication reminders or assistance, use of medical equipment, and chronic condition support; complete required training, demonstrate competency, and promptly report changes in client condition to the supervisor or RN.
Qualifications
- Must meet Colorado Medicaid caregiver eligibility requirements
- Ability to pass required background checks and screenings
- Reliable transportation and ability to travel to client homes (if applicable)
- Ability to follow written and verbal instructions
- Must be 18 years of age or older and meet all eligibility requirements for employment
- Compassionate, dependable, and respectful demeanor
Physical Requirements
- Ability to stand, walk, bend, and lift up to agency-specified limits
- Ability to assist with transfers and mobility using proper body mechanics
Work Environment
- Client homes and community settings
- Exposure to varying home conditions
