Peralta Home Health Services is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team. We are looking for clinicians who enjoy working independently while making a meaningful impact on patients recovering in the comfort of their own homes.

What You'll Do
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop individualized treatment plans based on physician orders and patient goals.
- Provide skilled interventions including therapeutic exercise, gait training, balance training, strengthening, transfer training, and fall prevention.
- Evaluate mobility, functional limitations, pain, and safety within the home environment.
- Educate patients and caregivers on exercises, mobility techniques, adaptive equipment, and home safety.
- Monitor patient progress and modify treatment plans as appropriate.
- Complete accurate, timely documentation in the electronic medical record.
- Communicate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team.
- Supervise Physical Therapist Assistants (PTAs) when appropriate.
- Participate in case conferences and coordinate patient care.
Qualifications
- Current Arizona Physical Therapist license in good standing.
- Graduate of an accredited Physical Therapy program.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Current automobile insurance.
- Ability to travel to patient homes within assigned territory.
- Home health experience preferred but not required.
- Strong communication, documentation, and time-management skills.
