Responsibilities
- Remotely monitoring patient vital signs and health data using various devices .
- Analyze data trends to identify potential health risks or changes in patient conditions.
- Responding to alerts and notifications from monitoring devices.
- Provide patient education on the use of RPM technology and how to interpret their data.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Educate patients and their families about health management, disease prevention, and treatment options.
- Train patients on the use of RPM devices and applications.
- Answer patient inquiries and address concerns related to their monitoring program.
- Coordinate patient care through virtual consultations and communication platforms
- Maintain accurate and up-to-date patient records
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Strong understanding of remote monitoring technology and devices.
- Knowledge of medical records management and vital sign assessment.
- Strong communication skills to effectively interact with patients, families, and healthcare teams.
- Compassionate demeanor with a commitment to providing high-quality patient care.
- Knowledge of HIPPA regulations and patient confidentiality.
