Job Overview
We are seeking a dedicated and highly skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with expertise spanning various clinical settings, including acute care, long-term care, assisted living, and community health. As an LPN, you will play a vital role in delivering compassionate nursing services, supporting patient recovery and well-being through precise assessments, medication administration, and comprehensive care management. Duties
- Perform detailed patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections accurately, including IV infusions and catheterizations, following established protocols and safety standards.
- Document medical records meticulously using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to ensure compliance with HIPAA regulations.
- Assist with complex procedures such as wound suturing, tube feeding management, ventilator support, and sterile processing techniques.
- Provide specialized care for patients with conditions such as dementia, Alzheimer's disease, developmental disabilities, or those requiring hospice or memory care services.
- Support discharge planning, case management activities, and coordinate with multidisciplinary teams across inpatient and outpatient settings.
- Monitor patients' vital signs and physiological responses during acute pain management or emergency situations like triage or trauma cases at Level I or Level II trauma centers.
- Educate patients and families on health coaching topics including medication administration, nutrition, infection control, and post-acute care strategies.
Skills
- Proficiency with EMR/EHR systems such as Cerner or Athenahealth for accurate medical documentation.
- Extensive experience in diverse clinical environments including hospital medicine, ICU (Intensive Care Unit), PICU (Pediatric ICU), emergency room settings, and nursing homes.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (International Classification of Diseases), CPT coding (Current Procedural Terminology), and diagnostic evaluation procedures.
- Skilled in performing venipuncture, phlebotomy, specimen collection/processing, and sterile techniques for infection control.
- Ability to manage complex patient needs involving pain management, airway management, catheter care, dialysis support, and gastrostomy tube feeding.
- Experience working with individuals with disabilities or developmental challenges in home & community care environments.
- Excellent communication skills to collaborate effectively with healthcare teams and provide compassionate patient education across diverse populations including pediatrics and geriatrics.
