Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans.
- Administer medications, injections, and other treatments following strict protocols and medical documentation standards.
- Manage complex wound care, including dressing changes and infection control procedures, ensuring optimal healing outcomes.
- Assist with specialized procedures such as catheterization, tube feeding, and ventilator management within the home setting.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and long-term care strategies that meet regulatory standards.
- Educate patients and their families on disease management, medication administration, and safety protocols to foster independence.
- Maintain accurate electronic health records (EHR/EHR systems), ensuring compliance with HIPAA and confidentiality policies.
Skills
- Proven experience with Epic EMR systems or similar electronic health record platforms for efficient documentation.
- Strong background in critical care environments such as ICU or emergency medicine, with familiarity in acute pain management and infectious disease care.
- Expertise in working with diverse populations including geriatric patients, individuals with developmental disabilities, dementia or Alzheimer’s disease, and those requiring hospice or memory care services.
- Skilled in performing patient assessments, vital signs monitoring, specimen collection/processing, and medical documentation with precision.
- Knowledge of long-term care regulations and best practices for skilled nursing facilities and assisted living environments.
- Proficiency in IV insertion/infusion techniques, medication administration including injections and venipuncture.
- Ability to handle complex cases involving trauma center experience or post-acute care scenarios; familiarity with ICD coding (ICD-9/ICD-10) is a plus.
- Excellent communication skills for triage calls, patient education, and interdisciplinary collaboration; adept at managing medical imaging or laboratory procedures when needed.
