Job Summary : Responsible for implementing service plans, medical appointments, documentation forms and ensuring daily needs are being met.
Specific Duties:
- Be willing to accept varied work hours as assigned. Schedules are determined according to the needs of specific families. Changes in schedule to be mutually determined with Service Coordinator or Programs Coordinator.
- Responsible for carrying out the consumers’ and families’ wishes.
- Be willing to:
- Transport consumers in personal vehicle.
- Provide proof of valid driver’s license.
- Provide verification of vehicle insurance coverage.
- Be willing to receive training and/or supervision in performing the following duties (dependent upon the individual’s needs and service plan.)
- Overview of Disabilities
- Positioning, lifting, carrying
- Feeding Techniques
- Medications and Administration
- Universal Precautions
- Be certified in CPR and First Aid
- Responsible for documentation necessary for accurate billing.
- Responsible for oversight of the following:
- Medication Administration Records and controlled medication counts (as required by PCISP)
- Financial documentation/logs (as required by PCISP)
- Charts/Logs (e.g. Dietary, Health related tracking logs as required by PCISP)
- Be in good physical condition in order to carry out duties.
- Realize that a job description is a summation of duties, but not all inclusive of responsibilities. Assignments may be delegated, as necessary, by the Supervisor.
- The Consumer Manager will report to the Programs Coordinator / Service Coordinator and the Executive Director.
- Ensure that all of the required documentation, daily checklists, and budget sheets are completed, accurate, and turned in on time.
- Attend doctor and dental appointments when necessary.
- Act as the main communicator to the office for the consumer.
- Assist with purchasing groceries and household items as needed.
- Report changes in the individual’s needs and condition.
