A Resume that includes the following minimum credentials: (please provide copies of the following documents.)
a. Doctor of Physical Therapy degree from an accredited physical therapy school
b. Current valid license to practice physical therapy in the Commonwealth of Virginia
c. Current Specialty Certifications including current CPR Certification
For each proposed PT candidate, submit:

✅ Copy of DPT Degree (or diploma/transcript showing DPT)
✅ Copy of Current Virginia PT License
✅ Copy of Current CPR Certification
✅ Copies of any Specialty Certifications (Orthopedic PT, Neurologic PT, Sports PT, etc., if applicable)
✅ Completed Attachment 2 – National Practitioner Data Bank (NPDB) Form for that provider
Resume Requirements:
Title: Physical Therapist (Correctional Healthcare)
Location: Virginia Department of Corrections Facilities – Statewide
Schedule: Up to 40 hours/week, as needed
Requirements:
DPT degree
Active Virginia PT License
Current CPR Certification
Ability to pass criminal background screening
Experience in outpatient, rehab, SNF, hospital, or correctional settings preferred
Responsibilities:
Physical therapy evaluations and treatment
Care plans and progress notes
Documentation within 24 hours
Coordination with medical providers
Patient education and rehabilitation services
Locations to Serve:
� Haynesville Correctional Center

Service to be provided approximately 40 hours per week.
Population: 893 male inmates
Pay: $65.00 per hour
