As the Home Health & Hospice Licensed Practical Nurse you will provide oversight for personal care, homemaker/companion and private duty nursing care to promote maximum health and independence for clients. Incorporate appropriate resources and provide for continuity of care. Provide routine visits and treatments for clients/residents who have acute or chronic difficulties interfering with health, safety, and independence. Make visits on campus and in the community. Document care provided.
Shift Details:
MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:

Provide services in accordance with organizational policies and procedures and regulations that define scope of practice.

Respond to emergency calls from independent living residents.
-
Visit residents upon request for services, including on return from hospital stay or level of care change to determine the type personal care services needed. Assess and refer residents to skilled home health or other levels of care as appropriate.

Supervise personal care aids and nurse assistants in following individualized plans of care

Assist the RN in carrying out the plan of care
-
Assist the patient in learning appropriate self-care techniques.

Assess patient/family response to care
-
Ensure communication of information to appropriate team members. Notify physician and Clinical Coordinator of significant changes in client’s health status. Involve family members or other supportive persons as appropriate in assisting residents with their health care

Perform other duties as directed by supervisor

Comply with agency policies and procedures relative to infection control and safety
EQUIPMENT:
- Must have experience with all types of office equipment, i.e. copier, fax, multi-line phone system
PHYSICAL REQUIREMENTS:
- Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway; climb at least 2 flights of stairs and be able to stand and/or walk for a period of at least 4 hours
REQUIRED KNOWLEDGE, SKILLS AND ABILITY:
- Demonstrated ability to read, write and speak English.

Must possess good verbal and written communication skills • Must be able to meet deadlines, work under limited supervision.

Current CPR certification (Basic Life Support)

Valid Driver’s License in the State of Virginia with a minimum age 21 to drive for the organization

Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers • Proof of motor vehicle insurance
REQUIRED EDUCATION AND EXPERIENCE:
- At least one year of nursing experience in Hospice, Home Health or a Skilled Nursing Facility

Current and unrestricted license as a Licensed Practical Nurse in the Commonwealth of Virginia
