This position is responsible for the direction and regulatory compliance of the residential program.
ESSENTIAL FUNCTIONS AND BASIC DUTIES
- Coordinate all conferences relative to the Individuals programs with all team members and outside programs to ensure coordination of care.
- Develop and observe the implementation of Residential Habilitation Plans and approved Behavioral Support Programs.
- Ensure plans are carried out by staff utilizing prescribed actions.
- Check data to ensure accuracy and submit Res Hab billing documents to the Director of Residential Programs for billing.
- Observe techniques of service delivery and maintain monthly program notes.
- Review and update DDP 2’s as required
- Work closely with all clinicians to ensure that all needs are met.
- Work in conjunction with the Residence Manger to ensure corrections/changes are made when necessary to bring residence into full regulatory compliance.
- Coordinate all admission/discharge procedures.
- Train staff on res hab plans
- Works in coordination with the Residential Manager to ensure all Individuals attend daily activities. Checks community inclusion logs to ensure compliance.
- Ensure Person Centered Planning to live a richer life style.
- Provide all staff training on res hab plans, when new plans are developed.
- Ensure that all Individual paperwork is updated and completed as necessary which includes an annual CFA, Res Hab plan, Money management assessment/ PEP, annual self- medication assessment, MP, Psychotropic consent.
- CFAs should be updated and reviewed by the team at least twice annually or when changes are made. A copy is to be submitted to the Director of Residential Programs
o Observations will be as needed to ensure that the individuals we serve in the home are given choice and the ability to exercise their freedom, enhance independence and engage in self-management including during mealtime.
o Provide support and training to all staff on eVero system.
o Participate in state audits as part of the implementation team.
o Configure eVero and assist with building programs to meet organizational requirements.
o Develop reports as assigned.
o Perform additional job-related duties as requested by supervisor.
o Training and orient new users to the system.
o Manage new employee and Individual related changes consistent with policy for onboarding individuals’ and managing changes.
o Monitor essential elements for billing in the eVero system.
o Work with other departments such as Finance to ensure collaboration of the system.
o Requires regular travel to Catholic Charities locations.
o Performs other related duties as requested.
- All other duties necessary to provide continuous services to Individuals.
Primary Schedule: Part time – 12-16 hours weekly as dictated by the needs of the residence.
Qualifications: Minimum Bachelor’s Degree in Education,
Psych, Social, or in a related Social Services OR a degree in
Nursing as a Registered Nurse
Field; One-year experience in working
with Developmentally Disabled.
On – call 24/7 Must have own transportation
Probationary Period: 6 months
