Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Geriatric Case Manager in Naples, Florida.
POSITION OVERVIEW
- Community-based case management role serving older adults and their families
- Provide both short-term and long-term case management services
- Develop individualized care plans and coordinate community resources
- Conduct home visits and maintain ongoing client communication
- Help seniors remain independent and safely age in place
- Strong autonomy with support from a dedicated team
COMPENSATION
- Annual Salary: $55,000
- Fully Employer-Paid Health Insurance

SCHEDULE
- Monday-Friday Schedule
- 9:00 AM - 5:00 PM
- No Weekends
- No Call Requirements
- Community-Based Work Environment
ROLE
- Conduct client assessments and develop individualized care plans
- Coordinate healthcare, social services, and community resources
- Perform home visits and ongoing case management follow-up
- Advocate for older adults and their families
- Maintain documentation and care coordination records
- Support clients in maintaining independence and quality of life

Requirements:
- Bachelor's Degree in Social Work REQUIRED
- Master's Degree in Social Work Preferred
- Case Management Experience REQUIRED
- Experience Working with Older Adults Preferred
- Comfortable Conducting Home Visits REQUIRED
- Strong Communication and Organizational Skills REQUIRED
- Ability to Develop Individualized Care Plans REQUIRED
- Compassionate and Client-Focused Approach REQUIRED
