Job Summary
We are seeking a dynamic and experienced Head of Care Navigation to lead our patient-centered care coordination team. In this pivotal role, you will oversee the development and implementation of comprehensive care navigation strategies that enhance patient experiences, streamline discharge processes, and optimize resource utilization. Duties
- Lead and manage the care navigation team, including supervising staff responsible for patient assessment, discharge planning, and care coordination.
- Develop and refine care navigation protocols aligned with hospital standards, including NCQA (National Committee for Quality Assurance) standards and HIPAA (Health Insurance Portability and Accountability Act) compliance.
- Collaborate with clinical departments such as ICU, Emergency Medicine, Pediatrics, Behavioral Health, and Surgery to facilitate effective patient transitions across care settings.
- Oversee data collection and analysis related to patient flow, discharge efficiency, and utilization review to identify opportunities for process improvement.
- Ensure accurate documentation review, medical records management, and adherence to medical terminology standards in all care plans and reports.
- Coordinate with social work, case management, addiction counseling, and behavioral health teams to address complex patient needs holistically.
- Drive initiatives in discharge planning, patient assessment, and behavioral therapy to improve overall patient care experience and outcomes.
Requirements
- Proven experience in hospital settings with expertise in acute care environments such as Level I Trauma Centers or Level II Trauma Centers.
- Strong background in nursing or healthcare management with supervisory experience overseeing multidisciplinary teams.
- Extensive knowledge of hospital medicine, ICU procedures, emergency medicine protocols, pediatrics—including PICU—and home health or hospice care services.
- Familiarity with medical terminology, anatomy and physiology knowledge essential for effective case management and documentation review.
- Experience with managed care programs like Medicare and understanding of NCQA standards for quality assurance.
- Demonstrated leadership skills with the ability to motivate teams while managing complex workflows involving behavioral health, social work, and behavioral therapy services.
- Excellent communication skills paired with a thorough understanding of HIPAA regulations for data security and patient privacy.
- Ability to analyze data for utilization review purposes while supporting continuous improvement initiatives in patient care processes. Pay: $30.86 - $37.16 per hour
