Senior Care Solutions is a Medicare-certified home health agency dedicated to providing high-quality, patient-centered care in the comfort of our patients' homes. We are looking for a compassionate and motivated Occupational Therapist (OT) to join our growing clinical team.
The Occupational Therapist will provide skilled occupational therapy services to home health patients based on their individual needs and established plans of care. This position is ideal for an OT who enjoys working one-on-one with patients, promoting independence, and helping individuals safely perform daily activities in their homes.

Perform comprehensive occupational therapy evaluations in the patient's home.

Develop individualized treatment plans based on patient needs, physician orders, and clinical findings.

Provide skilled interventions to improve patients' ability to safely perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

Provide therapeutic exercises, functional mobility training, adaptive techniques, and home safety education.

Assess the patient's home environment and make recommendations for safety and equipment as appropriate.

Educate patients and caregivers regarding treatment plans, home exercise programs, safety, and strategies to promote independence.

Monitor patient progress and modify treatment plans as needed.

Communicate changes in patient condition and progress with the physician and interdisciplinary care team.

Complete all required clinical documentation accurately and within required timeframes.

Collaborate with RNs, PTs, STs, MSWs, HHAs, and other members of the interdisciplinary team.

Participate in care coordination and case conferences as needed.

Maintain compliance with Medicare, state, and agency requirements.

Provide compassionate, patient-centered care while maintaining professional standards.
Current Florida Occupational Therapist (OT) license in good standing.
Graduate of an accredited Occupational Therapy program.
Current CPR certification.
Previous home health experience preferred, but not required.
Strong clinical assessment and documentation skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage a flexible field-based schedule.
Reliable transportation and willingness to travel within the assigned service area.
Comfortable using an EMR system for clinical documentation.
Strong organizational skills and attention to detail.
