Role Overview:
As a Home Health Nurse at BrightStar Care of W. Volusia, you will be responsible for providing exceptional nursing care to our clients in their homes. In this role, you’ll collaborate closely with healthcare teams to develop and implement individualized care plans that cater to each patient's unique needs. Whether you work on-site, you’ll have the flexibility to contribute to making a significant difference in our clients' lives.
Key Responsibilities:
- Conduct comprehensive assessments of patients’ health needs.

Administer medications and treatments as prescribed by physicians.

Document patient care and progress accurately in electronic health records.

Educate patients and their families on health management and care protocols.

Coordinate with other healthcare professionals to ensure holistic patient care.
Available Shifts : 7pm-7am Friday, Saturday, Sunday
Qualifications:
You're a great fit if you have the following skills.
- Current RN License or LPN license in Florida.

Vent & Trach Experience

Excellent communication and interpersonal skills.

Ability to work independently and manage time effectively.

Compassionate and patient-centered approach to care.
Bonus If You:
- Have previous experience in home health or hospice care.

Possess CPR and First Aid certifications.

Are bilingual, particularly in Spanish or another language.
Employment Type & Availability:
This position is Full-Time, available on Friday, Saturday, and Sunday.
Location:
This position is on-site only in Middleburg, Florida.
