Access TLC is seeking a full-time Care Coordinator to join our team. The Care Coordinator will provide in-office case management and conduct regular field visits to patients. The ideal candidate will be responsible for coordinating care, and ensuring they receive the necessary services in a timely and efficient manner.
Access TLC support patients by arranging for community-based and other services that allow recipients to experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind and a genuine team player.
Position Responsibilities:

Conducting assessments, developing care plans, arranging for and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data.

Makes visits to patients to establish ongoing eligibility for the program, to monitor the safety and services identified in the Individual Service Plan and address the concerns of the patient.

The role includes occasional field visits to connect directly with participants, as well as monthly team meetings and training
Requirements:

Must have either a Bachelor or Masters Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, or Sociology.

Must have a California Driver’s License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements.

One year of related work experience. Intern or volunteer experience is also applicable.

Training will be in-person in our Moorpark office at 5401 Tech Cir. Moorpark, CA 93021
Job Type: Full-time Monday through Friday 8:30 am – 5:00 pm
Rate of Pay: $24.00 /hour
