Position Summary
This individual will make a difference in the lives of many by assisting people with intellectual challenges and developmental disabilities to access and coordinate needed support and services in community settings. Knowledge of social, economic, health or rehabilitative services objectives is important, as is knowledge of methods of compiling, organizing and analyzing data. This person must have strong problem-solving skills and the ability to plan and develop programs or program components. The case manager must evaluate and monitor service delivery and must possess the ability to provide technical assistance regarding program and client issues. An effective communicator, this person will be able to establish and maintain effective working relationships with others and to understand and apply applicable rules, regulations, policies and procedures.
Key Responsibilities
- Support individuals with intellectual and developmental disabilities in accessing and coordinating community-based services and resources.
- Facilitate development of individualized service plans (ISPs) by gathering assessments, reviewing clinical recommendations, and coordinating planning meetings with individuals, families, and care teams.
- Help individuals and families identify, access, and maintain cost-effective services, including Medicaid eligibility and required documentation.
- Monitor service delivery to ensure supports align with individual goals, needs, and preferences; conduct regular check-ins and annual plan reviews.
- Coordinate and arrange services, ensuring timely access to care and adherence to ISP outcomes.
- Maintain consistent contact (including monthly face-to-face visits) and document all services, progress, and interactions in compliance with organizational and regulatory standards.
- Respond promptly to concerns or crises, providing support and escalating issues as needed.
- Track eligibility, recertifications, and required clinical documentation to ensure compliance with deadlines and state requirements.
- Uphold confidentiality standards and ensure compliance with HIPAA, DBHDD, and organizational policies.
Qualifications
1. Minimum bachelor’s degree in human service field, such as, social work, psychology, special education, nursing, or closely related field required.
2. Minimum two years of experience serving persons with DD/IDD or related disabilities and two years’ experience in a healthcare, habilitative/ rehabilitative, behavioral health setting, such as ICF/IDD, psychiatric inpatient hospital/facility, specialty clinic or program, special education department, residential or similar setting required.
3. Mobile Worker: This is a mobile position, meaning that regular travel to different work locations is essential. Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
4. Reside in the same territory they are assigned to work in; exceptions may be considered, due to business need.
5. Occasional evening and weekend work may be required as job duties demand.

- Strong problem-solving skills with the ability to gather data, identify issues, and draw sound conclusions.
Work Environment
This is a mobile, field-based position requiring travel throughout the assigned counties. Work includes home visits, community settings, and occasional remote/administrative tasks. Flexibility for evenings or weekends may be required.
