Pleasant Valley NursingRehab Center is seeking a PRN Physical Therapist (PT) to join our rehabilitation team. This opportunity offers flexible scheduling while supporting residents with mobility, strength, and functional goals in a skilled nursing setting.
Responsibilities:

Complete physical therapy evaluations and provide individualized treatment

Address gait training, balance, strengthening, transfers, and endurance goals

Collaborate with rehab staff and nursing on resident care plans

Monitor progress and adjust treatment as needed

Maintain timely documentation
Qualifications:

Active PT license required
-
Skilled nursing experience preferred but not required

Strong clinical and communication skills
