Requirements
- 3+ years of clinical experience in acute care preferred, including areas such as:
- Internal Medicine
- Surgery
- Orthopedics
- Mother/Child
- Cardiology
- Oncology
- Experience in Rehabilitation, Home Health, or Long-Term Care may also be considered
- Strong Case Management experience
- Ability to perform comprehensive admission and discharge risk assessments
- Knowledge of quality, financial, payer benefits, utilization, and risk management
- Knowledge of Medicare, Medicaid, and commercial insurance requirements
- Knowledge of community resources
- Knowledge of regulatory and accreditation organizations preferred, including Joint Commission and DPH
- Strong organizational, analytical, and data-management skills
- Excellent communication, collaboration, negotiation, and interpersonal skills
- Ability to work independently and maintain confidentiality
- Ability to establish and maintain effective relationships with physicians, patients, families, insurers, and healthcare agencies
- Strong computer skills and ability to manage multiple applications
- BLS required – AHA or American Red Cross
- Active Massachusetts RN license required and must be in hand at submission
- CCM/ACM certification preferred
- RN-BC Case Management certification preferred
- Meditech Expanse experience highly preferred
- Must be willing to work five 8-hour shifts; 10- or 12-hour schedules are not available
Responsibilities
- Manage the healthcare needs of patients across the continuum of care
- Coordinate multidisciplinary services to achieve quality and cost-effective patient outcomes
- Review patients at admission and, when appropriate, prior to admission
- Conduct ongoing concurrent reviews throughout hospitalization
- Evaluate appropriate levels of care and utilization of healthcare resources
- Complete comprehensive admission and discharge risk assessments
- Develop and coordinate individualized transition-of-care plans
- Arrange appropriate post-hospital services, resources, and follow-up care
- Coordinate discharge planning with patients, families, physicians, nurses, insurance providers, and community agencies
- Monitor patient progress and modify care plans based on changing clinical and social needs
- Verify payer benefits, coverage limitations, and authorization requirements
- Identify barriers to discharge and develop solutions to facilitate timely transitions
- Advocate for appropriate patient resources and services
- Collaborate with interdisciplinary teams to promote quality, safety, patient satisfaction, and cost-effective care
- Maintain accurate and timely documentation of assessments, care coordination, utilization review, and discharge planning
- Analyze patient and operational data and prepare reports when required
- Participate in performance-improvement initiatives
- Maintain compliance with regulatory, accreditation, and organizational standards
- Protect patient confidentiality and handle sensitive information appropriately
Pay: $2,350.00 - $2,650.00 per week
