American Traveler is hiring a travel RN Case Manager for an acute inpatient care coordination role in Silver Spring, MD, requiring an active MD or Compact RN license and Epic experience.

Acute inpatient hospital setting focused on discharge planning and care coordination

Collaborates with hospital-based physicians, nursing staff, utilization review, and ancillary team members
-
Works with patients, families, and community providers to develop and coordinate safe discharge plans
-
Coordinates transitions of care with community providers and ambulatory case managers to prevent readmissions

Day shift: 8:30 AM – 5:00 PM (8-hour shifts)

Active RN license — Maryland or Compact license required and must be in hand at time of consideration (pending licenses not accepted)

Active certifications required at time of consideration (no pending certifications accepted)

Experience in case management, discharge planning, or care coordination in an acute inpatient setting

Responsible for overseeing the management and coordination of care for the acute inpatient population, including facilitating appropriate level-of-care transitions and discharge planning

Collaborates with rounding physicians, patients and families, nursing, and utilization review to ensure clinically appropriate and cost-effective care progression
