As a 1:1 Facility and Community Support Staff, you’ll play a vital role in the lives of individuals with Intellectual and Developmental Disabilities by promoting their health, safety, and overall well-being—both within our agency facility and out in the community.

Your responsibilities include:
- Ensuring the health, safety, and welfare of the individuals you support
- Offering guidance, instruction, and hands-on support in both community and facility settings
- Actively identifying and facilitating opportunities for community inclusion and engagement
- Helping individuals build interests, strengths, and skills that reflect their personal goals and dreams
- Completing timely, accurate documentation that reflects progress, challenges, and evolving needs

Essential duties and Responsibilities
- Utilize local community resources in order to promote the individual's community integration, independence, purpose, responsibility and interdependence.
- Provide services and programming to reach all individual’s abilities in their home and/or community according to their goals and objectives.
- Provide necessary current needs of the individuals based on their Service Plan.
- Perform accurate record keeping and documentation for the individuals served.
- Help individuals participate in their communities by researching and working with the individual to develop independence in the community.
- Assist individuals in becoming self-reliant and having a voice.
- Ensure the individual’s human rights, health; safety, welfare, and dignity at all times while enhancing the quality of the individuals’ everyday life are preserved.
- Assists individuals in becoming independent and building their own social networks.
- Collaborate with involved agencies, family members, natural supports, networks and other service providers independently and cooperatively with others, to foster the individuals’ independence and community integration.
- Seek out opportunities for individuals to participate in volunteer opportunities, community adult learning, training and social group opportunities.
- Provide individuals with opportunities within their community to develop community safety skills, as well as enrich their skills academically, socially, recreationally and artistically.
- Transport clients to community events, volunteering, health and fitness activities and appointments.
- Attend meetings as deemed necessary
- Will meet 24 hours of training per year.
- Adhere to the Department of Human Services and other regulatory entities.
- Works independently with minimal supervision.
Facility Essential Duties and Responsibilities:
- Adhere to the mission and Philosophy of CCCC.
- Provide Day and Community programming to reach all individuals abilities and community resources.
- Provide the necessary current needs of the individuals based on the Individual Support Plans.
- Provide services that meet the individual’s health, safety, and welfare needs.
- Perform timely and accurate record keeping and documentation on the individuals served.
- Maintain a safe clean working/program/transportation environment.
- Advocate for individuals with residential provider staff, parents, guardians, and administrators.
- Ensure that individual’s human rights, health, safety, welfare, and dignity are preserved.
- Assists individuals with direct care services which may include personal care needs such as eating, drinking, restroom and daily needs.
- Participate in the development and implementation of agency activity calendars to ensure individuals' outcomes are being met.
- Provide individual with opportunities to develop community safety skills, as well as enrich their skills academically, socially, recreationally and artistically.
- Assist the Program Specialist in preparing Individual Support Plans, program planning and assessments.
- Adhere to the rules and regulations of the Pennsylvania Department of Human Services, and the Department of Aging and other regulatory entities.
- Attend a minimum of 24 hours of training annually as well as completing mandatory trainings such as maintain certification of 1st Aid/CPR and Fire Safety.
- Attend scheduled staff meetings.
- Attend Incident Management training as scheduled and report incidents as a mandated
- Assist in transporting individuals, lifting/transfers and distributing medication to individuals
- Encourage teamwork with colleagues and individuals to promote a healthy work environment
- Follow Chain of Command in reporting issues and concerns and keep Director in the know concerning issues and significant program changes.
- Prepare and participate in program monitoring.
- Comply with all agency policies and procedures.
- Complete other duties deemed appropriate by the agency CEO.
- Adhere to all Federal, State, Local and agency guidelines regarding COVID-19 processes and procedures.

· Ability to work flexible hours and locations to meet program needs.
Education. Credential and Experience:
A high school diploma or GED equivalent
Special Requirements:
Must be 18 years or older
- Must have a valid driver’s license and reliable vehicle
- Maintain and submit vehicle insurance, registration and state inspection verifications.
- Must have a physical and TB testing completed prior to employment.
- PA State Clearance Background Check/FBI Background (if applicable)
- Agree to utilize the agencies timekeeping, communication and documentation software/systems
- Drug Test when applicable
