A leading healthcare organization is seeking a compassionate and dedicated Home Health Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in the home setting. Provide hands-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols
Deliver patient care as ordered, including IV infusions, infusion pump management, and tube feedings under the supervision of a Registered Nurse
Implement and maintain patient and caregiver education plans under the direction of a Registered Nurse
Monitor patient conditions and communicate treatment responses and changes in status to the supervising Registered Nurse
Administer medications and evaluate patient responses when appropriate
Collaborate with interdisciplinary team members to ensure continuity and quality of care
Maintain accurate and timely documentation of patient assessments and care provided
Promote patient safety, independence, and positive health outcomes in the home environment
Graduate of an accredited Practical Nursing program
Current Licensed Practical Nurse (LPN) license in the state of practice
Current Basic Life Support (BLS) certification
Valid Florida driver's license or ability to obtain one within 30 days of hire
Associate Degree or Diploma in Nursing required
Minimum of one year of Licensed Practical Nurse experience required
Previous home health or home care experience preferred
Strong communication, organizational, and patient education skills
Ability to work independently and effectively manage patient visits
Must meet all applicable background screening requirements
