The Department of Medicine is seeking a compassionate and patient-centered Patient Navigator to support individuals living with sickle cell disease throughout their healthcare journey. In this impactful role, you will serve as a trusted resource for patients by coordinating care, responding to phone calls and patient inquiries, facilitating timely follow-up appointments, and helping patients navigate the healthcare system. You will work closely with patients to address insurance-related barriers and connect them with available resources, while collaborating with physicians, clinic staff, and multidisciplinary care teams to ensure seamless, high-quality care.
