Assessment/Reassessment:

An ongoing process which is continuous for the duration of the patient's stay.

Interviews patient and other individuals to collect information.

Observes physical condition including physically examining the patient.

Identifies economic, educational, and cultural barriers; patient values and beliefs; spiritual influences; and age specific and biophysical needs according to accepted standards of practice.

Reviews all available information, including medical records, patient history, and other documentation.

Documents all findings and observations.

Initiates reassessment of patients as necessary when a patient is currently or potentially at physiologic, psychological, and psychosocial risk.

Recognizes deviations from baseline and interprets subtle changes in a patient's condition.
Planning:

Formulates nursing care plans based upon assessment and in accordance with provider orders, procedures and/or protocols.

Identifies nursing care objectives.

Prioritizes patient care needs and activities as a member of an interdisciplinary patient care team.

Coordinates patient care in collaboration with other interdisciplinary patient care team members.

Establishes patient goals and treatment plans.

Modifies plans of patient care based on reassessment findings, specific patient care needs, and/or changes in interdisciplinary treatment plan.

Assists in discharge planning needs.

Identifies appropriate tools and/or instructing methods for patient/family education.
Implementation:

Provides individualized and specialized nursing care and administers medications and treatments to patients as prescribed.

Observes patient's response to interventions and treatments and monitors for side effects of medication administration.

Recognizes and responds to changes in patient status timely and appropriately. Intervenes when situations call for immediate action.

Communicates identified needs and patient responses to members of the interdisciplinary team.
Evaluation/Monitoring:

Observes, evaluates, and communicates the effects of treatments and interventions.

Reassesses and revises plan of care in collaboration with other members of the health care team.

Communicates with patients and care givers regarding changes in patient care and treatment plans.
Applicants are required to meet ALL of the following at the time of application filing:
1. Have a current and active license to practice as a Registered Nurse issued by the California Board of Registered Nursing.

2. Current Basic Life Support (BLS) for Healthcare Providers (CPR & AED) Program certification issued by the American Heart Association.
~AND~
3. Graduation from the Nursing program of the Los Angeles County College of Nursing and Allied Health (CONAH) within the last two (2) years.
Applicants are to attach a legible copy/image of evidence of program completion from CONAH, such as a diploma, certificate, or "official" transcripts, to their application at the time of filing or emailing the document/s within 7 calendar days from filing the application online to the Exam Analyst at [email protected]. Applications submitted without the required evidence of completion will not be accepted.
License(s) and Certificate(s) Required:
- This examination requires applicants to hold an active Registered Nurse license issued either by the California Board of Registered Nursing. Be sure to enter all required license information in the Certificates and Licenses section of the application.
Applicants are required to attach a legible photocopy/image of the required license and BLS certificate from the American Heart Association to the application at the time of filing or within 7 calendar days from the application filing date. Applications submitted without the required evidence of BLS certification will be rejected as incomplete.
Required certification(s) and/or licenses have to be active and unrestricted, or we are unable to accept your application. Additionally, to receive credit for certification(s) and/or licenses in relation to any desirable qualifications, the certification(s) and/or licenses have to be active and unrestricted.
Please ensure the Certification/License Section is completed. Provide the title(s) of your required license(s), the number(s), date(s) of issue, date(s) of expiration and the name(s) of the issuing agency for each certification/license as specified in the Requirements.
The original required license to practice as a Registered Nurse issued by the California Board of Registered Nursing and BLS certification will need to be presented during the selection process and/or prior to a start date being established.
Other License Information – Possession of a valid California Class C Driver's License or the ability to use an alternative method of transportation when needed to carry out essential job functions.
Physical Class III – Moderate: This position requires standing or walking most of the time with bending, stooping, squatting, twisting, reaching, working on irregular surfaces, occasional lifting of objects weighing over 25 pounds, and frequent lifting of 10-25 pounds.
This opportunity is considered noncompetitive, meaning you will only need to meet the requirements. If you do, we will put you on a list with everyone who qualifies. Candidates entitled to veteran’s credit will be placed ahead of candidates who are not entitled to veteran’s credit. Available shift:
