Full Job Description

Full‑time Registered Nurse Case Manager in a home health setting near Spokane, WA. Responsible for coordinating comprehensive patient care, developing individualized care plans, and supporting patients and families throughout their healthcare journey.

Key Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans

Coordinate and manage patient care across the continuum, ensuring quality and efficiency

Provide skilled nursing care in accordance with physician orders and established protocols

Monitor patient progress and adjust care plans as necessary

Educate patients and families on disease management, medications, and care techniques

Collaborate with physicians, therapists, social workers, and other healthcare professionals

Maintain accurate and timely documentation in compliance with regulatory standards

Ensure compliance with all state, federal, and agency guidelines

Qualifications:

Active Registered Nurse (RN) license in Idaho or compact license
Associate or Bachelor of Science in Nursing (ASN or BSN) required
Minimum of 1–2 years of nursing experience; home health or case management experience preferred
Current BLS (Basic Life Support) certification
Valid driver’s license and reliable transportation

Preferred Skills:

Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and manage time effectively
Experience with electronic medical records (EMR) systems
Knowledge of Medicare/Medicaid regulations in home health
