SUMMARY OF POSITION RESPONSIBILITIES
Under general supervision of the RN Supervisor, the LVN will perform detailed initial assessments of program clients, present clinical information to the NP/psychiatrist, manage front desk triage, and complete all required paperwork.
GENERAL DUTIES
The duties specified below are representative of the range of duties assigned to this position and are not intended to be an inclusive list.
1. Renders nursing care to clinic patients.
2. Perform new client intakes; complete the clinical portion of the psychiatric assessment;
complete all required forms requiring the client’s signature/consent.
3. For previously seen clients, update the chart as needed.
4. Present current and background clinical information to the psychiatrist/np for medication evaluation.
5. Facilitate appropriate referrals for clients needing follow-up care.
6. Administer medications/injections and TB tests as needed.
7. Complete all required chart paperwork in compliance with the City & County policies and
document services fully and accurately.
8. Using the appropriate billing codes, submit billing for all services performed daily.
9. Attend weekly staff meeting.
10. Adhere to all Westside Policies and Procedures.
11. Other duties as assigned by the Supervisor.
LICENSES/CERTIFICATION
- Possession of a valid Licensed Vocational Nurse license issued by the California Board of Nursing.

OTHER SKILLS & ABILITIES
1. One (1) year of experience working directly with chronically mentally ill individuals
preferred.
2. Knowledge of nursing/patient(client) care theory to assess patient (client) care
needs; nursing and health care management theory to set goals, objectives and priorities.
3. Ability to identify and analyze problems, gather relevant data, implement appropriate
action and evaluate the effectiveness of decisions and maintain accurate records.
SCHEDULE
Time and days are varied as determined by Program Director; Weekends & Holidays may be required.
