As a Navigator in the Participant Wellness & Safety Program, you will work directly with individuals experiencing homelessness, substance use disorders, and complex health challenges. Through trust‑based relationships and low‑barrier engagement, you will connect participants to critical services like healthcare, treatment, and housing stabilization. The goals of this position are to prevent overdose and overdose deaths and to stop the spread of communicable diseases. This role will work one-on-one with individuals who are actively using opioids and other substances. In addition, the Navigator will build internal capacity for Avivo teams through training, consultation, and other creative methods.

Build trusting, consistent relationships with participants using trauma‑informed, person‑centered approaches

Connect individuals to healthcare, MAT/MOUD, Naloxone and overdose response training, and treatment services

Coordinate referrals and “warm handoffs” with internal teams and community partners

Support participants in developing and working toward self‑directed recovery and wellness goals

Develop crisis plans and provide de‑escalation and crisis intervention as needed

Conduct outreach and engagement with individuals experiencing homelessness, including those not connected to services

Host or support HIV and Hepatitis C testing events and connect participants to follow‑up care

Maintain accurate and timely documentation in electronic health record systems and required reporting tools

Collaborate across teams and partners to improve access to services and reduce barriers

Contribute to a culture of dignity, safety, and equity in all participant interactions

High school diploma or GED
-
Minimum of two years of experience for single adults, experiencing homelessness or similar experience. Experience should also include developing and delivering culturally responsive services for individuals living with SMI and/or SUD.
- Understanding of trauma‑informed care, harm reduction, and recovery‑oriented approaches (or willingness to learn)

Knowledge of community resources, housing systems, and behavioral health supports

Strong relationship‑building skills with the ability to engage participants in a non‑judgmental, person‑centered way

Experience using electronic health records and tracking participant data

Ability to manage documentation, communication, and multiple priorities in a fast‑paced environment

Strong communication and collaboration skills

Lived experience is valued and may be considered as part of experience requirements
