Responsibilities
- Assess patients’ physical conditions by reviewing medical histories and conducting detailed physiotherapy evaluations
- Develop and implement tailored care plans that incorporate evidence-based practices such as trigger point therapy, orthopedics, and geriatrics interventions
- Guide patients through therapeutic exercises to improve strength, flexibility, and mobility while ensuring proper technique
- Coordinate discharge planning and facilitate smooth transitions between inpatient, outpatient, and home & community care settings
- Utilize EMR (Electronic Medical Record) systems to document assessments, treatment progress, and discharge instructions accurately
- Collaborate with multidisciplinary teams including physicians, nurses, and occupational therapists to optimize patient outcomes
- Provide education to patients and their families on injury prevention, physiotherapy techniques, and home care routines
Qualifications
- Bachelor’s or Master’s degree in Physiotherapy or a related field from an accredited program
- Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction
- Strong knowledge of anatomy, physiology, medical terminology, and care plan development
- Experience working in acute care hospitals, outpatient clinics, or home health environments is highly preferred
- Proficiency with EMR systems for documentation and patient management
- Demonstrated expertise in discharge planning, patient care across inpatient and outpatient settings, and community-based rehabilitation
- Familiarity with specialized techniques such as trigger point therapy and orthopedics treatments
- Excellent communication skills for effective patient education and interdisciplinary collaboration
