Licensed Social Worker
Job Summary:
We are seeking a compassionate and experienced Social Worker to join our team at White Oak Rehabilitation and Nursing at Divine in Toledo, OH. As a Director of Social Worker, you will play a vital role in providing patient-centered care, addressing the social and emotional needs of our patients, and collaborating with our interdisciplinary healthcare team to achieve optimal outcomes.
Responsibilities:
Assess and address the social and emotional needs of patients, including those related to mental health, substance abuse, and traumatic experiences
Develop and implement individualized treatment plans, utilizing evidence-based practices and state-of-the-art interventions
Provide counseling, education, and support to patients and their families to promote coping and resilience
Collaborate with the interdisciplinary healthcare team to develop and implement comprehensive care plans
Participate in patient evaluations, assessments, and progress reports
Maintain accurate and up-to-date records of patient interactions and treatment plans
Provide crisis intervention and emergency services as needed
Stay current with changes in social work practices, relevant laws and regulations, and best practices in patient care
