Physical Therapist – Home Health
Compensation: Pay-Per-Visit/Hourly Hybrid
30,000 Sign-On Bonus (in six increments in three years)
Territory: Walnut Creek to Conchord
Productivity Expectation: 30 Points Weekly
Mileage Reimbursement: $.725 Per Mile Between Visits

Territory
This position serves patients throughout the North Bay market. Patient schedules are designed to be geographically efficient, and clinicians are not usually expected to cover the entire territory in a single day.
Essential Responsibilities
- Provide skilled physical therapy services in accordance with physician orders and the established plan of care
- Evaluate patient functional status, mobility, strength, gait, range of motion, and rehabilitation potential
- Develop and implement individualized treatment plans
- Educate patients and caregivers regarding treatment goals, therapeutic exercises, safety, and use of adaptive equipment
- Communicate patient progress, treatment response, and changes in condition to physicians and the interdisciplinary team
- Participate in care planning, case conferences, and discharge planning activities
- Complete clinical documentation and progress summaries in accordance with company policy
- Identify the need for additional home health services and coordinate appropriate referrals
- Supervise Physical Therapy Assistants in accordance with state regulations and company policy
- Provide consultation and education to caregivers and other healthcare team members as appropriate
Qualifications
- Graduate of an accredited Bachelor's, Master's, or Doctor of Physical Therapy program
- Current California Physical Therapy license in good standing
- Minimum of two years of Physical Therapy experience preferred
- Home Health experience preferred but not required
- Current CPR certification
- Excellent communication, organizational, and clinical skills
- Valid driver's license, reliable transportation, and current automobile insurance

Pre-Employment Requirements
- Current Tuberculosis (TB) Test or Chest X-ray
- Titers or proof of immunity for Varicella, MMR, and Hepatitis B
- Proof of Tdap vaccination
- Proof of annual Influenza vaccination
- Current physical examination
