Responsibilities
- Assist individuals with activities of daily living (ADLs), including personal care, hygiene, dressing, and meal preparation.
- Support individuals in achieving personal goals and increasing independence.
- Document services and complete daily progress notes accurately.
- Accompany individuals to medical appointments, community outings, and recreational activities.
- Maintain a safe, clean, and supportive living environment.
- Follow all agency policies and state regulations.
- Communicate effectively with supervisors, coworkers, families, and healthcare professionals.
Qualifications
- Must be authorized to work in the United States.
- High school diploma or GED.
- Valid New York State driver's license preferred (or required for positions involving transportation of individuals).
- Reliable transportation to travel to assigned worksites throughout Suffolk County.
- Ability to successfully complete a background check and all required pre-employment screenings.
- Previous experience as a Direct Support Professional (DSP), Caregiver, Home Health Aide (HHA), Personal Care Aide (PCA), Certified Nursing Assistant (CNA), or in a similar direct care role is preferred, but not required.
