Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
- Monitor and record patient vital signs including blood pressure, pulse, temperature, and respiration rates using EMR (Electronic Medical Records) systems
- Support medication administration following medical protocols and home health regulations
- Observe and document patient condition changes, ensuring accurate medical records are maintained confidentially in compliance with HIPAA guidelines
- Assist with specialized care needs such as catheter care, dementia and Alzheimer’s care, memory care, and end-of-life support
- Safely transfer patients using patient transfer methods like Hoyer lifts or other assistive devices while adhering to safety standards
- Provide companionship and emotional support to promote mental well-being and client communication
Requirements
- Valid certification as a CNA (Certified Nursing Assistant), PCA (Personal Care Aide), or HHA (Home Health Aide) with current licensure or registration where applicable
- Proven experience working with geriatric patients or individuals with developmental disabilities or disabilities requiring specialized care such as dementia or Alzheimer’s disease
- Knowledge of medical terminology, medication administration procedures, and vital signs monitoring
- Familiarity with EMR/EHR systems for documentation and reporting purposes
- Ability to perform heavy lifting safely and effectively using proper patient transfer techniques.
- Strong understanding of home health care regulations, HIPAA compliance, and patient confidentiality standards
- Current CPR/First Aid certification and basic life support training preferred
- Excellent communication skills with the ability to connect empathetically with clients and team members
