Care and Dignity Services (CNDS) has been established since 2017.
We are currently hiring a Group Home Program Coordinator (PC) for our DDD group homes located in the Laveen and Mesa area. The PD is considered a safety sensitive position and ensures smooth day-to-day operations of the group home programs, supports a high quality of life for those individuals in residence and that everyone's needs are supported within the group home. This is a great opportunity for a dedicated professional experienced with the Division of Developmental Disabilities (DDD) to provide leadership and support to our Direct Support Professional staff and the people we serve and support.
- Supervise House Managers and Direct Support Professionals (DSP) in the completion of their daily routines, implementing programming utilized in the home.
- Participate in the hiring process for House Manager and Direct Service staff.
- Review integral parts of tasks which provides services to clients, as well as critical communication regarding client services.
- Ensures programs meet licensing/survey requirements and contractual compliance. Including responsible for assuring that the group home meets the agency’s standards for cleanliness and appearance.
- Responsible for coordinating with the Schedulers to ensure the group homes maintain proper staff to client ratio.
- Report and assist in the creation and revision of agency policies and procedures that align with DDD requirements.
- Attend administrative meetings. Utilize critical thinking skills and collaborate with peers to find win-win solutions.
- Responsible for carrying out various assignments in community relations which may include communication with schools, medical personnel, employment resources, etc.
- Investigate all incident reports.
- Participate in the Intake and Discharge process.
- Complete and maintain administrative requirements including the creation and execution of program strategies and goals and monitor outcomes. Complete required documentation accurately and within designated timeframes.
- Ensures that clean and safe environments are maintained and that support provided meets expectations of all policies and procedures; complies with all AZ DDD and other relevant licensing requirements.

Pre -Employment Requirements:

- At least two years of management job experience (consecutive).
- Employment experience with working with adults and children of DDD populations, familiar with Arizona State law regarding DDD population.
- Class One Fingerprint Clearance Card
- AZ Driver License and Driver License history
- Notarization of Criminal Disclosure Completion of I-9/SAM
- Social Security Card
- Central Registry Background Check
- CPR/First Aid certification
- Article 9 certification
- Prevention and Support certification
- A reliable means of transportation
