Position Summary:
Aventura Skilled Nursing Facility is seeking an experienced MDS Coordinator (LPN/RN) to lead resident assessments and ensure accurate, timely MDS submissions. This key role supports quality care, regulatory compliance, and interdisciplinary care planning.

Qualifications:
Active LPN or RN license (in good standing)
Minimum 2 years of MDS experience in long-term care
Strong knowledge of RAI process, PDPM, and reimbursement
Detail-oriented with excellent organization and communication skills
Team player with ability to work independently
Key Responsibilities:
Complete and submit accurate MDS assessments (MDS 3.0)
Ensure compliance with CMS and state regulations
Lead care plan meetings and coordinate IDT collaboration
Oversee CAAs and individualized care plans
Audit documentation to support coding accuracy
Track assessment schedules and maintain compliance
Support audits, surveys, and quality improvement initiatives
