Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure their comfort and dignity
- Support patients with mobility and transfers using appropriate techniques such as patient transfer methods and manual handling tools like Hoyer lifts
- Monitor vital signs and observe patient conditions to detect changes or concerns promptly, maintaining detailed care plans
- Administer medications accurately following prescribed instructions while adhering to HIPAA regulations to protect patient privacy
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking to promote healthy living
- Provide transportation assistance for medical appointments, errands, or community activities ensuring safety at all times
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
- Document patient progress, communicate effectively with healthcare teams, and update care plans regularly
- Support clients with specialized needs including Alzheimer’s care, dementia care, memory care, autism support, developmental disabilities, or neurodevelopmental disorders
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes or home health environments is highly preferred
- Demonstrated knowledge of patient observation techniques and monitoring vital signs accurately
- Hands-on experience with medication administration, first aid, CPR certification, and emergency response procedures
- Familiarity with care for individuals with dementia, Alzheimer’s disease, or other neurodegenerative conditions
- Experience working with diverse populations including seniors, individuals with disabilities or developmental challenges is advantageous
- Strong communication skills for effective client interaction and collaboration with healthcare professionals
- Ability to perform heavy lifting safely and operate equipment such as Hoyer lifts or patient transfer devices when necessary
