Job Summary
We are seeking a dedicated and highly organized Long Term Care Scheduler & Transport Coordinator. Coordinates nursing and caregiver shifts to ensure facilities meet patient care and regulatory requirements. Responsibilities include managing the daily calendar, filling last-minute call-outs, communicating with field staff, and adjusting schedules to changing patient volumes. Also oversees patient transportation logistics within our healthcare facility. The ideal candidate will possess strong coordination skills, attention to detail, and the ability to manage complex scheduling requirements efficiently. This role involves coordinating transportation services for long-term care residents, ensuring timely and safe transfers, and maintaining seamless communication among staff, patients, and transportation providers. The successful candidate will play a vital role in enhancing patient care delivery through effective transportation management and scheduling.
Duties
- Schedule Creation & Maintenance: Develop and post master schedules for RNs, LPNs, and CNAs to guarantee mandated staff-to-patient ratios are maintained.
- Attendance Tracking: Manage requests for Paid Time Off (PTO), leaves of absence, and vacations while ensuring adequate shift coverage.
- Crisis Management: Act swiftly to fill sudden call-ins, no-shows, and emergency gaps
- Compliance & Reporting: Monitor labor hours and track staffing data to ensure adherence to state and federal healthcare regulations
- Staff Communication: Serve as the primary point of contact for caregivers regarding their shift preferences, schedule changes, and availability.
- Develop and maintain detailed transportation schedules for long-term care residents, ensuring timely pickups and drop-offs aligned with medical appointments and care plans.
- Coordinate with healthcare staff, patients, families, and external transportation providers to facilitate smooth and efficient transfers.

Pay: $24.65 - $29.68 per hour
