Job Overview
We are seeking an experienced, organized, and detail-oriented MDS Coordinator to join our skilled nursing facility in Maryland. The MDS Coordinator is responsible for managing the Resident Assessment Instrument (RAI) process, completing accurate and timely MDS assessments, and supporting individualized resident care planning.
This position works closely with nursing leadership and the interdisciplinary care team to ensure compliance with CMS requirements, Maryland regulations, and facility policies. The ideal candidate has strong clinical assessment skills, comprehensive knowledge of Medicare reimbursement, and experience in a skilled nursing or long-term care setting.
Key Responsibilities
- Coordinate and oversee the complete MDS and RAI process in accordance with CMS guidelines, Maryland requirements, and facility policies.
- Complete and submit accurate MDS assessments within all required timeframes.
- Develop and maintain an assessment schedule to ensure timely completion of all required assessments.
- Review residents’ medical records, nursing documentation, therapy notes, physician orders, diagnoses, medications, and supporting clinical information.
- Collaborate with nursing, rehabilitation, social services, dietary, activities, physicians, and other members of the interdisciplinary team.
- Facilitate care plan meetings and help develop individualized, resident-centered care plans.
- Ensure care plans accurately reflect each resident’s needs, risks, preferences, goals, and services.
- Monitor Medicare Part A residents, skilled coverage, assessment reference dates, and required documentation.
- Support accurate reimbursement under the Patient-Driven Payment Model (PDPM).
- Review documentation for accuracy, consistency, medical necessity, and appropriate clinical support.
- Identify documentation concerns and provide education to nursing staff and other departments.
- Monitor quality measures and assist with Quality Assurance and Performance Improvement initiatives.
- Maintain resident confidentiality and comply with HIPAA requirements.
- Participate in audits, surveys, utilization reviews, and additional compliance activities.
- Remain current on changes to CMS guidance, the RAI Manual, Medicare requirements, and Maryland long-term care regulations.
Qualifications
- Current, active RN license in Maryland required; compact-state licensure may be considered when legally applicable.
- Previous MDS Coordinator experience in a skilled nursing or long-term care facility required.
- Thorough understanding of the RAI process, MDS 3.0, PDPM, Medicare, Medicaid, and care planning requirements.
- Knowledge of CMS requirements and Maryland regulations governing skilled nursing facilities.
- Experience using an electronic medical record system; PointClickCare experience is strongly preferred.
- Strong clinical assessment, documentation review, and critical-thinking skills.
- Excellent organization and time-management skills with the ability to meet strict assessment deadlines.
- Effective written and verbal communication skills.
- Ability to collaborate successfully with residents, families, clinical staff, leadership, and the interdisciplinary care team.
- Working knowledge of HIPAA, infection prevention, resident rights, and quality-measure requirements.
