Position Summary
The Physical Therapist provides skilled, patient-centered care in the home setting and uses clinical expertise to support mobility, safety, independence, and quality of life. This role is ideal for a licensed professional who values meaningful patient relationships, clinical autonomy, and collaboration with an interdisciplinary care team.

Requirements:
- Current Physical Therapist license in the State of Arizona

Graduate of an accredited Physical Therapy program

Current CPR certification

Valid driver’s license, reliable transportation, and automobile insurance that meet agency requirements

Strong clinical judgment, organization, and communication skills

Ability to work independently while collaborating effectively with an interdisciplinary team

Home health experience is preferred
