Responsibilities:
- Assist individuals with activities of daily living (ADLs)
- Encourage independence and community participation
- Follow individualized care plans
- Complete documentation accurately
- Provide compassionate, person-centered support
Qualifications:
- Minimum 6 months of direct care or patient care experience
- High School Diploma or GED required
- Must be able to complete OPWDD clearance requirements
