The Position: Licensed Practical Nurse (LPN) – Assisted Living – Per Diem
Location: Flushing, NY
Schedule: Per Diem / As Needed
Weekend shifts: 7:00 am–5:00 pm
About the Role: As a Per Diem Assisted Living LPN, you will provide resident-centered nursing support while helping maintain the health, safety, and well-being of our residents. Responsibilities may include clinical observation, medication administration, documentation, communication of changes in condition, and coordination with the clinical team.
This opportunity is ideal for an LPN seeking additional hours, supplemental income, or a flexible schedule while continuing to provide meaningful care to residents in an Assisted Living environment.
Because this is a per-diem position, the ideal candidate is dependable, clinically confident, adaptable, and comfortable working independently while recognizing when changes in a resident's condition need to be communicated or escalated.

What we are looking for:
- Current NYS Licensed Practical Nurse (LPN) license.
- Prior nursing experience in assisted living, long-term care, rehabilitation, home health, or a related healthcare setting is preferred.
- Experience working with adult and/or geriatric populations is preferred.
- CHHA/Home Care experience is a plus.
- Strong clinical assessment and observation skills.
- Experience with medication administration and clinical documentation preferred.
- Ability to recognize and appropriately communicate changes in a resident's condition.
- Ability to work independently with minimal supervision.
- Excellent written and verbal communication skills.
- Dependable, adaptable, and comfortable working in a per-diem environment.
- Basic computer skills and ability to use electronic documentation systems.

Pay: $38.00 - $40.00 per hour
