The Emergency & Transitional Housing (ETH) Program is dedicated to helping individuals rebuild their lives after incarceration or involvement with the justice system by providing safe transitional housing, compassionate support services, and pathways toward long-term stability and independence. Through comprehensive onsite services—including case management, vocational training, housing advocacy, medical and behavioral health care, and mental health support—the program empowers participants to overcome barriers, strengthen life skills, and successfully reintegrate into the community.
Reporting to the Program Manager of Case Management, the Case Management Supervisor plays a vital leadership role in fostering a supportive, healing-centered environment at the Fresh Meadows, Queens program site. This position provides administrative and operational oversight while guiding a multidisciplinary team committed to delivering high-quality, person-centered care to justice-impacted individuals with complex needs.
The Case Management Supervisor will supervise a team of 3–4 case managers, oversee care coordination efforts, support quality improvement initiatives, facilitate team collaboration, and ensure participants receive compassionate and effective services tailored to their goals and strengths. Working closely with senior leadership and interdisciplinary providers, the supervisor helps cultivate a program culture rooted in dignity, harm reduction, accountability, and hope.

Key Responsibilities
Program Leadership & Operations
- Lead the day-to-day operations of onsite case management and housing placement services.
- Collaborate with program leadership to implement systems, workflows, and policies that enhance service delivery and participant outcomes.
- Help create a safe, respectful, and supportive program environment where participants can heal and grow.
Participant Advocacy & Support
- Ensure participants have access to critical resources including healthcare, housing, employment, education, and behavioral health services.
- Support the development of individualized housing and service plans that reflect each participant’s goals and strengths.
- Assist in resolving participant concerns, crisis situations, and behavioral interventions with empathy and professionalism.
- Maintain a small caseload of high-need participants when necessary.
Care Coordination
- Partner with internal and external providers, family members, and support systems to strengthen continuity of care and promote long-term stability for participants.
Staff Supervision & Team Development
- Provide supervision, mentorship, and professional development to case managers and support staff.
- Participate in hiring, onboarding, evaluations, and team-building initiatives that foster a collaborative and inclusive workplace culture.
- Lead onboarding and ongoing trainings that strengthen staff knowledge, skills, and confidence.
Quality Improvement & Compliance
- Monitor program quality and compliance through regular audits, reporting, and chart reviews.
- Use data and feedback to identify opportunities for continuous improvement and stronger participant outcomes.
- Ensure services align with internal standards, regulatory requirements, and mission-driven values.
Qualifications
- Bachelor’s degree in social services, psychology, social work, or a related field preferred.
- Master’s degree and active NYS LMSW, LCSW, or LMHC license strongly preferred.
- Minimum 3 years of experience in case management or direct client services.
- At least 1 year of supervisory experience.
- Experience supporting individuals impacted by the criminal legal system, homelessness, substance use, or co-occurring disorders.
- Strong communication, documentation, and leadership skills.
- Commitment to harm reduction, trauma-informed care, and strengths-based approaches.
- Ability to work collaboratively across multidisciplinary teams.
- Passion for advancing equity, dignity, and opportunity for underserved communities.
