NATURE OF WORK: Would you like to be the lead in implementing and providing an exciting new Public Health and Wellness Program on the Skokomish Reservation located in rural Washington State? This position will be responsible for extending care and wellness efforts for our patients beyond the clinic walls. Activities will include, but not be limited to: providing health and wellness education and outreach and community training; assisting with phased program design and development; coordinating and/or conducting COVID-19 screening and testing; coordinating a variety of vaccination clinics; and participating in COVID-19 and emerging public health concerns prevention, planning and response efforts both on the reservation and within the local area; ensuring all efforts are culturally appropriate; and similar. Long-range plans include the implementation of a home health care program and other new and enhanced services. This position will supervise and work in coordination with a Wellness Outreach Coordinator and other team members.
The successful candidate will have a degree in Public Health and/or hold a Washington State Medical Doctor, Advanced Registered Nurse Practitioner, Family Nurse Practitioner, Registered Nurse or Practical Nurse license.
JOB DUTIES:
- Ensuring effective care management that includes medically-related as well as social wellbeing aspects as detailed in case management plans and/or directed by providers.
- Coordinating testing, screening, vaccination and similar care outside of the clinic.
- Assisting with program implementation and planning for future expansion with a focus on self-sustainment following a three-year start-up.
- Conducting initial and ongoing community health and wellness assessments.
- Serving as the liaison between clinical staff and external programs and services including those provided by the Tribe, local government, state and/or federal entities.
- Conducting extensive and effective community outreach and education in coordination with the Wellness Outreach Coordinator that will include; hosting events, providing presentations, attending functions, coordination training, classes and similar.
- Working in coordination with assigned Case Managers and others for those individuals with special needs to ensure quality care and healthcare management.
- Documenting activities in the Electronic Health Record (EHR) system as well as databases and other as required.
- In conjunction with clinic staff, overseeing the ordering and proper storage /maintenance/documentation for vaccines as required by the State Department of Health and other entities' guidelines. Coordinating the administering of vaccines and ensuring that EHR immunization package and other databases are kept up-to-date.
- Coordinating comprehensive, culturally acceptable personal and public health that are available and accessible for community members.
- Supervising the Wellness Outreach Coordinator and other individuals as directed by the Health Director.
- And other duties as requested by the Health Director.

QUALIFICATIONS:
Bachelor's degree in public health or affiliated field and/or a Washington state Medical Doctor, Advanced Registered Nurse Practitioner, Family Nurse Practitioner, Registered Nurse or Practical Nurse license. A combination of education, experience and skills may be considered if deemed adequate by the Health Director.
Minimum of two years of supervisory experience (required).
Minimum of two years of successful program management (preferred).
Experience in program and/or strategic planning (preferred.)
Experience coordinating or managing a clinical and/or public health program (preferred).
Public speaking and presentation experience (preferred).
Prior experience working with Native American Indian population (preferred).
Experience providing community education, outreach and events (preferred).
Experience developing marketing and training materials (preferred).
Ability to multi-task while meeting competing deadlines (required).
Ability to effectively communicate both in writing and orally (required).
Approved results of a pre-employment drug screen and fingerprint background check (required).
Valid Washington State driver’s license and proof of automobile insurance (required).
