Full-Time/Part-Time| Field Work in Bronx I Brooklyn-Based | $25/hr
Chemlu Developmental Disabilities is expanding its work in the Bronx through New York’s Social Care Network (SCN), a Medicaid initiative connecting families to food, housing, transportation, and health-related social supports.
We are seeking two high-energy Outreach & Social Care Screener/Navigators who are comfortable working in the community, building relationships, and conducting on-site screenings with Medicaid families.
This role is outreach-driven. If you are motivated, personable, enjoy meeting people and building community connections- speaking with families, landlords, and community organizations, this may be a strong fit. Must be proactive, motivated by results and comfortable with working outside of a traditional office setting.
What You Will Do
- Conduct in-person screenings with Medicaid members in the Bronx
- Build relationships with community partners, landlords, and local organizations
- Table at community events and outreach days
- Educate families about available supports including meals, housing assistance, and transportation
- Complete eligibility screenings and navigation documentation
- Follow up with families to ensure services are delivered
- Maintain accurate reporting and service documentation
Who We Are Looking For
- Strong communicator who is comfortable approaching people and starting conversations
- Organized and able to manage follow-ups
- Comfortable working both independently and in the field
- Experience in community outreach, social care screening case management, care coordination, or social services preferred
- Bilingual (Spanish) preferred
- Ability to travel to Bronx locations as needed. Position on-the-road with some hours from home
Compensation
- $25 per hour
- Full-time position
- Hybrid flexibility, but must be available for in-person Bronx outreach. This position is an on-the-road position with some hours from home
