Our Value Based Care department is looking for a full time Manager to join their team! The hours are in office, Monday - Friday, 8-4:30 pm.
Summary:
The Program Manager, Value Based Care position leads quality improvement and value-based care initiatives by participating in quality outcomes initiatives, coordinating quality improvement (QI) efforts including development, implementation, data collection and analysis, and assisting with projects as assigned around Value Based Care. This role educates and communicates incentive program regulations, workflows to achieve required program goals and opportunities to improve care team efficiencies.
Essential Functions include, but are not limited to:
Practice Performance Facilitation (Practice Improvement Facilitator – PIF): Leads practice transformation initiatives by working closely with practice sites to analyze, implement, and optimize evidence-based workflows and operational processes. The role focuses on improving quality measure performance, increasing preventive and chronic disease management, reducing unnecessary emergency department and inpatient utilization, and supporting value-based care programs that improve outcomes while reducing the overall cost of care.
- Assist clinical teams in the gathering and analysis of outcomes information using outcomes management systems; ensures use of appropriate analytical techniques and strategies.
- Respond to requests for outcomes data from internal and external sources.
- Help to gather data and prepare reports for National Committee for Quality Assurance (NCQA), other accrediting/regulatory agencies, and internal QI initiatives.
- Help to plan, implement, design, and maintain group-wide QI and utilization management programs.
- Provide support to the Clinical Applications Support Team.
- Obtain/maintain working knowledge and proficiency in Community Care Physicians' information systems, including the following applications: practice management, electronic health record, and reporting tools.
- Help to identify/devise appropriate data survey collection tools and techniques.
- Participate in QI research, data collection and analysis, and documents findings.
- Assist with analysis of clinician and physician profile reports from third-party payers.
- Use statistical, economic, and financial principles to prepare reports, assemble, and validate data.
- Travel to Practice Sites, as needed.
- Perform related work as required.
Compensation: $65,500 - $85,000 per year

Requirements:
