JOB SUMMARY: The Case Aide under the direction of the Assistant Program Director will be responsible for promoting effective community reintegration for individuals who have traditionally been unable to manage in less supervised settings. As part of our multidisciplinary team the case aide will work closely with clinical staff to assist individuals with on-site rehabilitation and reconnection to community supports. Responsibilities include direct care, ILS assessment; escorting to appointments; co-leadership of skill development and socialization groups; and, participating in clinical team meetings.
WORK SCHEDULE: The Case Aide position is full-time. Hours worked will be Tuesday – Saturday or at mutually agreed-upon schedule. Generally 9-5 with at least one evening a week. Flexibility around schedule is required. The office location for this position in Brooklyn, New York.
SALARY AND BENEFITS: This is an hourly, non-exempt position that is paid an hourly rate of $22.50 USD and an overtime rate of $33.75 USD. Benefits include 14 paid holidays, 3 weeks of vacation, 4 personal days, 12 sick days, early closure at 12pm proceeding Monday agency-observed holidays. RESPONSIBILIITES:
Client/Service Delivery
· Participate in program related activities as directed by the Program Director.
· Evaluate ILS skills and needs of each individual, and make recommendations to Case Managers and Clinical Team
· Help implement individual service plans for each individual in concert with the Case Managers.
· Provide direct service in areas such as money management, daily living skills, and behavior management.
· Maintain records as required by the Case Managers, Clinical Team and Program Director.
· Submit records as required by the Case Managers, Clinical Team and Program Director.
· Participate in staff meetings and case conferences.
· Assist in setting up appointments and providing transportation for individuals.
· Support tenants as needed in recertifying benefits.
· Support unit inspections, conducting wellness checks, and assisting with work order access.
· Participate as a team member in planning, implementation, and evaluation of services provided.
· Facilitate and conduct tenant workshops and groups on a weekly basis, as well as lead and attend field trips as needed.
· Create and post program calendars monthly, as well as posting and maintaining accurate building signage.
· Assist with completing monthly inventory at the sites.
· Follow all policies and procedures relative to this position,
· Any other duties as assigned.
Team/Staff Relations
· Work as a Providence House team member with all staff and volunteers in order to implement the mission and core values of the agency.
· Attend regular internal and external agency meetings.
· Work as part of team to plan and develop residents’ programs.
Administrative
· Keep accurate, up-to-date documentation on all clients. Including but not limited to, treatment plans, case notes, etc.
· Complete all paperwork in a timely and accurate manner that ensures payment to the agency from various sources.
· Assists with any and all data collection systems.
REQUIREMENTS
· Associate’s Degree in Mental Health/Human Services Preferred.
· Minimum requirement: High School Diploma/GED, with at least two years of experience working with the formerly homeless, mentally ill population.
· Experience with long-term homeless, low income and diverse populations.
· Excellent verbal and written communication skills and the ability to work independently.
· Competent technology management skills, including proficiency with using Microsoft Office; and knowledge of client database management.
