Job Summary
We are seeking a dedicated and highly skilled Registered Nurse to join our skilled nursing team. The ideal candidate will possess a comprehensive understanding of patient care within a skilled nursing setting. As a Registered Nurse in our facility, you will be responsible for delivering exceptional nursing care, coordinating treatment plans, and ensuring compliance with healthcare standards. Your role will be vital in promoting patient well-being through compassionate, evidence-based practices and meticulous documentation.
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to develop individualized care plans.
- Educate patients and families on health coaching, discharge planning, medication administration, and chronic disease management to promote recovery and independence.
- Ensure compliance with HIPAA regulations and maintain accurate medical records in accordance with NCQA standards and ICD coding guidelines (ICD-9/10).
