Pay: $18 to $22/hr.
POSITION SUMMARY
The Diabetes Prevention Program Lifestyle Coach is responsible for providing support and guidance to participants in the YMCA’s Diabetes Prevention Program and implementing the standard curriculum designed for the program. In addition, every position in the YMCA of Greensboro is responsible for carrying out our promise to strengthen the foundations of community with focus on Youth Development, Healthy Living and Social Responsibility while upholding our values of caring, honesty, respect and responsibility.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Teaching method: Deliver curriculum to class participants in effective, meaningful and compelling ways while encouraging individual goal-setting, group participation and interaction using open-ended questions. Facilitate commitment and retention of participants. Make learning a shared responsibility for the group.

Relationships: Create a motivating environment that is friendly and non-competitive in which you can foster relationships with and between participants.

Class prep and follow-up: Prepare before each class by reviewing the lesson plan, content for class, participants tracking data, and by sending reminder communications to participants. Arrive on time, dressed appropriately. Be accessible to participants before and after sessions to answer questions and provide follow-up. Follow up after class with participants who were unable to attend and offer an in-person make-up session.

Record-keeping: Record weekly data for participants within 48 hours of the class. Adhere to the YMCA’s HIPAA policies and procedures and ensure participants’ protected health information is secure using all required platforms for recording and tracking participation and progress including but not limited to CRM Management tools, ERH HIPAA Secure Participant Platforms and Grant Required Data Management Tools and Platforms

Team engagement: Actively participate in all Lifestyle Coach team discussions and professional development opportunities, offering ideas and tools for facilitation to other team members and remaining engaged at all times. Participate in quality assurance opportunities to support fidelity to the curriculum and to meet performance standards.
SUPERVISORY RESPONSIBILITIES

Education/experience. Associate degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience. Public health background a plus. Coaches with bilingual responsibilities must be fluent in the languages needed for the program.

Ability to read and interpret documents such as safety rules, operation and maintenance instructions and procedure manuals. Ability to effectively present information in one-on-one and small group situations to customers, participants and other employees of the organization. Ability to define problems, collect data, establish facts and draw valid conclusions.

Must demonstrate a commitment to and support of the Y’s mission and more specifically, the vision and goals of the Y’s focus on health seekers. Must have strong communication, organization, and relationship-building skills; the ability to build community within a group; good attention to detail; and empathy for health seekers. Must be flexible to work with people from different backgrounds and walks of life and in a wide variety of settings. Should be comfortable with data and possess strong computer skills. Cultural competency and understanding of behavior change are required. Familiarity and proficiency with Listen First or motivational interviewing highly encouraged.

While performing the duties of this position, the employee will regularly be required to talk or hear; and will occasionally be required to life and move up to 10 pounds.

While the position involves both in-person and virtual training, there are no specific vision abilities required beyond the ability to see and function in a virtual setting to lead the required sessions.

The noise level in the work environment is usually moderate.

Certifications: CPR/AED, First Aid, Blood borne pathogen training required after hire and must remain current. HIPAA certification required but may be obtained after hire.
